The deprescribing rainbow

The deprescribing rainbow highlights the clinical, psychological, social, financial and physical determinants that should be considered when deciding to undertake an episode of deprescribing. This framework is informed by literature on patient-centered care for older people and people with multimorbidity. A hypothetical case example showing polypharmacy in an older adult is presented.

Management of hyperglycaemia in type 2 diabetes

A consensus report by the American Diabetes Association (ADA) and the European Association for the Study of Diabetes (EASD) provides recommendations for patients with clinical cardiovascular disease, suggesting earlier use of a sodium–glucose cotransporter-2 (SGLT2) inhibitor or a glucagon-like peptide-1 (GLP-1) receptor agonist. For patients with chronic kidney disease or clinical heart failure and atherosclerotic cardiovascular disease, an SGLT2 inhibitor with proven benefit is recommended.

Omega‐3 fatty acids for the primary and secondary prevention of cardiovascular disease

A Cochrane review of 79 trials has concluded that increasing EPA and DHA has little or no effect on mortality or cardiovascular health. Low‐quality evidence suggests ALA may slightly reduce CVD event and arrhythmia risk. Effects on stroke are unclear.

Omega‐6 fats for the primary and secondary prevention of cardiovascular disease

A Cochrane review of 19 RCTS has found no evidence that increasing omega‐6 fats reduces cardiovascular outcomes other than MI, where 53 people may need to increase omega‐6 fat intake to prevent 1 person from experiencing MI. Although benefits of omega‐6 fats remain to be proven, increasing omega‐6 fats may be of benefit in people at high risk of MI. Increased omega‐6 fats reduce serum total cholesterol but not other blood fat fractions or adiposity.
