Forum: Polypharmacy and deprescribing

jar with capsules

All my working life as a pharmacist has been centred around improving patient outcomes with medication. Informally we have all been addressing the issues of polypharmacy and deprescribing for many years. This article written for the NHS(UK) is an easy to read insight into the main issues, financial, clinical implications, guidance and the tools which we can use in practice. It is relevant to our practice in Australia and a good starting point to help us consider a more formalised approach.
